Eligibility

If you:

• Live in Rochdale, Middleton or Heywood
• Have and individual budget or Individual Service Fund (ISF). Please click here for more information
• Have a mental health condition, mild to moderate Learning Disability (including people with ASD) or those with a dual diagnosis (including substance misuse)
• Are able to live independently but require some housing support with cooking, budgeting, development of social skills, or the supervision and monitoring of your health and well being
• Are aged over 18
• Have a CPN, social worker, probation office, care co-ordinator or similar
• Receive a Disability Living Allowance (DLA)
